Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.
The goal of our practice is to provide you with dedicated and friendly service. With that aim in mind, we will cater to all of your vision needs, answer any questions you might have and supply you with the most up-to-date information in order to keep your eyes clear and healthy. We are committed to excellence in serving your family's complete eye care needs.
Below are New Patient Registration forms you'll need to complete on your first visit. You can expedite matters by downloading these forms, completing them at home and handing them to our staff when you arrive for your initial visit to our office. Also, remember to bring your insurance card and an ID with you to your first appointment.
Whether your first appointment is for a checkup, a consultation, or a second opinion, we do all we can to make your visit a welcoming, informative, and productive experience. Once we gather the relevant background information, examine your eyes and obtain any diagnostic images, we’ll discuss if treatment is required and explain your best options in care.
